Laid Down:     January 3 1961
Launched:      December 15 1961
Commissioned:  November 9 1963
Decommissioned April 29 1993
Fate:          Stricken April 29 1993
               Sold to Australia September 17 1993 and 
			   cannibalized for spare parts.
